Forms must be keyed in all CAPITAL LETTERS.
 
 
“Key what you see” means to literally key what you see. You are trying to replicate the form as it was submitted; you cannot fix errors.


H24b_Place_Of_Service

Five character string. The first character can be alpha or numeric. All other characters are numeric.

Two to six character alpha numeric field
Key what you see. Space between modifiers is optional.

When the Procedure and Modifier codes are set on one line, space between them is optional.
